In November 2021, allegations of bullying, harassment, discrimination, and inappropriate conduct by an executive were leveled toward TSM CEO and co-founder Andy "Reginald" Dinh, prompting an investigation by Riot Games and an internal investigation by TSM. A Twitlonger by Reginald on the morning of May 27 confirmed that TSM's internal investigation of him had concluded and that the investigation had found no evidence of inappropriate or unlawful behavior by the CEO.

In Reginald's Twitlonger, he outlines the process TSM went through to conduct an investigation, which he recused himself from until after its conclusion. Reginald wrote that after speaking with 30 employees and contractors, past and current, including all current League of Legends players signed by TSM, the investigation had been concluded with no evidence that confirmed the allegations against him.

Reginald went on to admit that he would be working with an executive coach to improve his communication skills in the workplace and that he would be more mindful of how his communication could have an effect on his employees and overall company culture.

He also stated that TSM would be launching a handful of initiatives, including a three-month evaluation of company culture, implementing an anonymous reporting hotline for anyone to share concerns in the workplace, and facilitating consistent and ongoing employee workshops with outside experts. Reginald also included the full investigation summary at the end of the Twitlonger.

TSM has been a hotbed for investigations in 2022, including one into League of Legends coach Zhang "Peter Zhang" Yi for financial irregularities that resulted in his termination when the investigation began on March 18.

TSM, in conjunction with Dexerto's Richard Lewis, have also made the results of the international investigation of Peter Zhang public. By TSM's internal investigation, Peter Zhang did tamper with player salaries, allegations he had previously denied.
